Healthy Futures provides care right at your child’s school, including:

• Dental exams

• X-rays

• Cleanings

• Fluoride treatments

• Sealants

• Fillings (if needed)

Who can enroll?
All students are eligible. We accept Medicaid, commercial insurance, and uninsured students.
There is no cost to families, even if your child is uninsured.

How to enroll:
Submit a consent form anytime during the school year. To ensure your child is seen at the next clinic, please enroll as soon as possible.

• Pick up a paper form in the school office 

• Return completed paper forms to the school nurse or to the Healthy Futures email: healthyfutures@heartlandhealth.org
