USD 367 Substitute Teacher Information

For New Substitutes:

First of all, you must complete a Substitute Teacher Informational Form. Once you have been accepted as a substitute teacher, the following items are required to complete your personnel file:

Copy of Social Security Card
Copy of Driver's License
Completed W-4 Form
Completed K-4 Form
Completed Health Form
(T.B. Skin Tests can be done at the Miami County Health Department, 1201 Lakemary Drive, Paola, KS 66071 on Tuesdays (8:00-4:30) and Wednesdays (4:30-6:30). It costs $10.) Health Form Information
Copy of Teaching Certificate/Emergency Substitute Certificate
Completed Loyalty Oath (The central office has a Notary.)
Completed I-9 Form
Signed Drug Free Form
Completed Direct Deposit Agreement
Completed Ethnicity Form
